Insurance Plans

Eyecare Norman accepts all private pay patients and is contracted with most vision and medical insurances (including Medicare and Medicaid).  Before your appointment, feel free to contact us to verify we accept your specific insurance plan, and you can contact your insurance provider to determine plan specific coverage and eligibility.

Before your exam we will attempt to verify your insurance coverage and obtain authorization from your insurance carrier.  A quote of eligibility is not a guarantee of payment and the patient is responsible for any out of pocket expenses not covered by your insurance. Out of pocket expenses are due at the time services are rendered and include co-payments, deductibles and co-insurance. 

Please remember to bring your vision and medical insurance card(s) and a photo I.D. when you arrive for your exam. 

Office Forms

If your want to save time on appointment day, we will e-mail you a link to our office forms to be completed beforehand, or you can fill them in our office before your exam. Please show up 15 minutes earlier than your appointment if you plan on completing the paperwork in our office.
